Office locations are scattered throughout the rural areas, including: Delta, Gananoque, Kemptville, Perth and Prescott; with the main office located in Brockville.

LGMH is a registered charitable organization, predominantly funded and supported by the South-East Local Health Integration Network, Ministry of Health and Long-Term Care, Ministry of Attorney General and United Way of Leeds and Grenville.
